___ Audiocassette tape recorder. ALC switch on! (Last cleaned _____/____/___, by _____.)

___ Two lapel microphones with dual wire adaptor. Remember to turn both microphone switches to off when you are finished; no red should be showing on the two battery cases!!
OR
___ One omnidirectional microphone with wire and adaptor. (Place this between you and the narrator; rest it on the foam padding provided; it has no on/off switch.)

___ (____#) Blank 60-minute tapes for the interview (the back of the tape cassette should still have its plastic tabs); bring twice as many tapes as you think you'll need.

_x_ Electrical cord for recorder is built in; turn over the recorder and open the larger door.

___ 6' extension cord.

___ Six size C batteries (if possibly needed).

___ Camera with film in it (preferably, black and white).

___ Oral history release agreement form (and extra forms). (Must be signed by both.)

___ Biographical questionnaire form. (To be completed by narrator.)

___ Standard text for formal introduction to oral history tape. (For interviewer's use.)

___ "Please do not interrupt; interview in progress" form for interviewer to place on door.

___ Oral history interview abstract form. (Interviewer completes this after the interview, to enable future access to the collection.)

___ Deed of gift form. (For donations of collections to the SW Center.)

___ Reproduction permission form. (If a person loans us collections for copying.)

___ Brochure about the Center of Southwest Studies. (To give the interviewer.)

___ Commonly asked questions about donating. (A descriptive pamphlet to give to potential donors.)

___ Paper. (For taking notes during and after the interview.)
